Green Card Replacement
If you are a lawful permanent resident, you must replace your green card if:

It is expired or will expire within six months
-
Your previous card was lost, stolen, mutilated, or destroyed
-
You received your card before your 14th birthday and you are now 14 (unless your card is valid until your 16th birthday)

You have been a commuter and now will take up actual residence in the United States
-
You were a permanent resident residing in the United States and you now will take up commuter status
-
Your status has been converted into permanent resident status (e.g., agricultural worker)

You have a previous version of the alien registration card and now require the current Green Card
-
Your green card contains incorrect information
-
You legally changed your name or other biographic information on the green card
-
You never received the previous green card that USCIS issued
If you are a conditional permanent resident, you can only file the Form I-90 under the following circumstance (you would need to file the I-751 to remove the conditions):

Your previous card was lost, stolen, mutilated, or destroyed

Your card contains incorrect information
-
You legally changed your name or other biographic information on the card
-
You never received the previous card that USCIS issued
To replace your Green Card, you must file Form I-90, Application to Replace Permanent Green Card either online or by mail. If your application is approved you will receive a new Green Card in the mail.
